Board Petition
Pursuant to Executive Order 202.26 candidates for school board must self-nominate. While no signatures are required for self-nomination, the following required information must be contained within the filing such that the eligibility of the candidate may be verified by the district.
Items Needed
All items need to be submitted via email or drop off by appointment only to the District Clerk, hmartin@clake.org or (716) 753-5808 on or before 5 p.m. on Monday, May 11, 2020.
To help district voters make an informed decision at the polls on June 9, 2020, we are asking all Board of Education candidates to respond to the following questions and submit their responses with their completed Board petitions. This information will be published in a newsletter and mailed to all residents of the Chautauqua Lake Central School District. We also need a recent photograph. Please contact the CLCS District Office by Monday, May 11, 2020, with your questions or to arrange to have a photo taken.
Please respond to the questions on a separate sheet of paper or email your responses to hmartain@clake.org. All responses combined must total no more than 500-600 words. If there are questions you do not wish to answer, the response that will be published will be "no answer offered by the candidate."

3. What particular experiences or skills qualify you to serve as a school board member?

6. Are/were you the parent of child(ren) in the Chautauqua Lake Central School District? If so, please give name(s), grade(s), and approximate dates attended.
The Expenditure Statement form may be used by the Board of Educations whose total election expenditures so not exceed five hundred dollars ($500). If expenditures exceed $500, an itemized statement must be filed in accordance with Sections 1528 and 1529 of the Education Law.
