In an effort to continue to keep our Chautauqua Lake community informed, please see the information below regarding the immediate availability of the Pfizer booster shot. The Pfizer booster will be available at the Chautauqua Lake Vaccination Clinics for anyone that meets the requirements depicted below. This information came directly from our local Department of Health. Initial vaccinations for Pfizer and Johnson & Johnson are also still available.

Vaccination clinics are scheduled on Tuesdays through October from 4:00 to 7:00 p.m. at the Chautauqua Lake Central School bus garage. If you have any questions or need additional information, please contact our Local Department of Health at (716) 753-4312.

Vaccination clinics will continue to be made available here at the bus garage on Tuesdays through October 26, 2021.  
In order to be eligible to receive a booster dose of Pfizer vaccine tonight, you must have received two primary doses of Pfizer vaccine, with the second dose received before March 28, 2021 (6 months prior).  You also must meet one of the following conditions:
1.     Age 65 years or older, OR
2.     A resident of a long-term care facility age 18 years or older OR
3.     Age 50 through 64 years with one or more of the following conditions due to increased risk of moderate or severe illness or death from the virus that causes COVID-19:
    a.     Cancer (current or in remission, including 9/11-related cancers)
    b.     Chronic kidney disease
    c.      Pulmonary disease, including but not limited to COPD (chronic obstructive pulmonary disease), asthma (moderate-to-severe), pulmonary fibrosis, cystic fibrosis, and 9/11 related pulmonary diseases
    d.     Intellectual and developmental disabilities including Down syndrome
    e.     Heart conditions, including but not limited to heart failure, coronary artery disease, cardiomyopathies, or hypertension (high blood pressure)
    f.       Immunocompromised state (weakened immune system) including but not limited to solid organ transplant or from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, use of other immune weakening medicines, or other causes
    g.     Severe obesity (BMI 40 kg/m2), obesity (body mass index [BMI] of 30 kg/m2 or higher but < 40 kg/m2)
    h.     Pregnancy
    i.       Sickle cell disease or thalassemia
    j.       Type 1 or 2 diabetes mellitus
    k.      Cerebrovascular disease (affects blood vessels and blood supply to the brain)
    l.       Neurologic conditions including but not limited to Alzheimer's disease or dementia
    m.  Liver disease; OR
4.     Ages 18 through 49 years with one or more of the underlying medical conditions listed above, based on individual benefits and risks; OR
5.     Ages 18-64 years who are at increased risk for COVID-19 exposure and transmission because of occupational or institutional setting based on individual benefits and risks.
