In order to be vaccinated individuals must be electronically registered no later than midnight on Sunday, February 13th. Registration links are below. Anyone who is a minor will also be required to bring a signed consent form with them, or send it prior by email.  A copy of the consent is attached.

The Department of Health will be back on March 8th for the 2nd dose. 

*If parents wish to accompany their child to the clinic, please be at the school by 12:15 pm.
