Parental Request

New York State Education Law 3012-c allows parents and legal guardians of a student to request the composite effectiveness scores and final ratings of teachers and principals to whom the student is assigned for the current school year.

This information may be released only to parents or legal guardians, and the district must verify the identity of the requestor.  Additionally, the information is to be provided only to the requesting parent(s) or legal guardian(s).  It is not to be shared with any other person.

Requests for this information must be made in writing using the request form available below.  All requests must be submitted by mail or in person to the Superintendent’s Office, Chautauqua Lake Central School, 100 N Erie Street, Mayville NY 14757.

Reports will be provided in person only.
